[11/10/15]   Common Causes of HIP pain:
Common causes of hip pain we see as physicians are hip arthritis and labral tears, trochanteric bursitis (trochanteric pain syndrome), and sacroiliac joint dysfunction.
Pain within the hip joint tends to be associated with arthritis or a tear of the hip labrum. Patients typically describe pain in the groin or front of the thigh. It can be worse with activity, painful when getting up from a chair, or uncomfortable when seated for long periods of time. These problems can be treated with stretching/strengthening the hip and core muscles, taking anti-inflammatory medication, and sometimes hip injections or even surgery is indicated.
Trochanter bursitis (commonly called “hip bursitis”) tends to cause pain on the side of the thigh and hip. It can be tender to touch or painful when lying on the hip. Generally, a stretching program focused on the abductor muscles and IT band with anti-inflammatories can be beneficial. Occasionally surgery is necessary when other treatments have failed.
Sacroiliac joint (SI joint) dysfunction causes pain around the low back and buttock region. Typical treatment protocols include stretching/strengthening/physical therapy as well as anti-inflammatory medication.
[10/29/15]   3 of the most common injuries in runners are patellofemoral pain (pain around the knee cap), meniscus tears, and tendonitis.
Patellofemoral pain is associated with increasing mileage and a change in running shoes. Improve patellofemoral pain by avoiding hills, taking anti-inflammatory medication, and strengthening the thigh muscles such as the quadriceps.
Meniscus tears are commonly described as catching, locking, and pain around the knee with twist activities (cutting/pivoting). Limiting activities and anti-inflammatory medication are the hallmark of non-operative therapy. Sometimes surgery is required to remove the torn tissue.
Tendonitis can be caused by overuse and due to inflammation. Stretching, ice, and eccentric exercises can treat this disorder.

It’s always a pleasure to discuss how surprised patients were following their knee replacement. As an orthopedic surgeon, I discuss patient’s fears and concerns about undergoing a knee replacement on a daily basis. As it turns out, my patient’s knee pain is one of my top concerns as well.

By using a variety of techniques we are able to minimize your pain after surgery. We call this a “multimodal pain protocol” with the aim of limiting narcotic pain medication (e.g. oxycodone, Vicodin, Percocet, or Norco). By decreasing pain before it happens and the need for narcotics after surgery patients are can have a speedy recovery.
Skin Crease ‘Bikini’ Incision for Anterior Approach Total Hip Arthroplasty: Surgical Technique... What is the "Bikini" Incision Anterior Hip Replacement, and why I use it?

This muscle-sparing technique for hip replacement uses an incision parallel to the hip crease and follows Langer's Lines. In contrast, a standard Anterior hip replacement incision runs down the thigh towards the outside of the knee and crosses Langer's lines.

A hip replacement incision in the bikini line has the added benefit of lower skin tension and improved scar appearance without sacrificing the surgeons view during hip replacement surgery.

[09/26/15]   "Pulled hamstring" or "hamstring strains":
It seems like hamstring injuries tend to occur more often in the preseason or first few weeks of the season. It may be related to decreased training in the off-season. Other risks include increasing age, poor conditioning, and previous hamstring injuries.

While some hamstring injuries require surgery, most can be treated effectively with R.I.C.E. therapy.

Prevent injuries by preparing for the season early, working on eccentric muscle strengthening, and sprinting exercises. The Nordic Hamstring exercise has been shown to decrease hamstring injuries in soccer players by 70% and may be worth implementing into a preseason workout program.

- An injury to the ligaments that hold the two leg bones together.

Will it need surgery?
- Low-grade injuries, without evidence of instability on x-ray, may be treated without surgery.
- Higher grade injuries may need surgery with one of a variety of techniques to "reduce the syndesmosis". This is a controversial topic regarding not only who to fix but, how to fix them.
